Job Description
Primary coverage in ACUTE REHAB and secondary ICF/SNF EVALUATE AND RE-EVALUATE ASSIGNED RESIDENTS/PATIENTS; PLAN AND IMPLEMENT AN ONGOING THERAPEUTIC PHYSICAL THERAPY PROGRAM AS PRESCRIBED BY THE ATTENDING PHYSICIAN AND/OR CONSULTING SPECIALIST AND PROVIDE CONSULTING PHYSICAL THERAPIST SERVICES. MUST BE AVAILABLE TO WORK SHIFTS WEEKENDS AND HOLIDAYS AS NECESSARY BASED ON HOSPITAL utilized in the unit/ - MeditechRequired skills and minimum years of 2 years work experience in at least 1 of the following settings working with adults (Acute rehab hosp. SNF)Specific licensures and certifications/registrations ; current State of Hawaii licenseShift or schedule -Fri 8:30am-5:00pm Tues-Sat 8:30am-5:00pm or Sun-Thurs 8:30am-5:00pm
