SUMMARY OF JOB RESPONSIBILITIES: The Clinic Registered Nurse is responsible for providing evidenced based nursing services to participants enrolled in the PACE program. The Clinic RN provides support to the participant and their caregiver/family members assists the participant to maintain or increase their health status through the provision of Clinic services including teaching counseling and other skilled interventions. This position is responsible for providing superior physical assessment techniques the development of effective nursing care plans and professional nursing care of the patient. The Clinic Registered Nurse must have well developed organizational and interpersonal skills to effectively coordinate the scheduling communication and encounter documentation of all care provided in compliance with regulatory requirements Requirements: Must have unrestricted Colorado/Compact RN license RN s with a minimum of two years experience in a clinic or med-surge setting RN s with strong communication skills and a desire to work in a team environment At HopeWest we hold strong core values that are followed internally with staff as well as in the daily care of our patients & their loved ones.
