Position Overview

The Case Coordinator will determine program eligibility and assess the need for care at home or in a personal care home for the elderly or infirm adult. He/she will coordinate the delivery of a broad range of professional and non-professional services based on the Case Coordinator Guidelines. Acts as a liaison and maintains communication with agencies and facilities involved with the client group.

Experience

  • Two years directly related community experience preferred.
  • Previous experience as a Case Coordinator in Community Health Services preferred.
  • Experience with EFT implementation considered as asset.

Consistent with the French Language Services Policy of the Government of Manitoba and the Winnipeg Regional Health Authoritys French Language Services Policy the WRHA is committed to offer services in French and English in designated program/service areas serving francophone communities in Winnipeg. Exprience

  • De prfrence deux ans dexprience dans la communaut directement en lien avec le poste.
  • De prfrence exprience antrieure titre de coordonnateur/coordonnatrice de cas dans des services de sant communautaires.
  • Une exprience de la mise en uvre dun programme dETP est considre comme un atout.

Education (Degree/Diploma/Certificate)

  • BSW OT PT BSc PN RPN BN RN SLP RD or related health/human service degree required.

  • B. Serv. soc. ergothrapie physiothrapie B. Sc. IPS inf. aux. aut. B. Sc. inf. inf. aut. orthophonie dittique ou diplme connexe en sciences de la sant ou en services sociaux requis.

Certification/Licensure/Registration

  • Valid drivers license and a vehicle.
  • Licensure/registration as per professional designation required.
  • If successful applicant is a Registered Dietitian must be a graduate of a recognized dietetic internship program accredited by Dietitians of Canada.

Accrditation/permis dexercer/inscription

  • Possder un permis de conduire valide et une voiture.
  • Titulaire dun permis ou dune autorisation dexercice dans sa profession.
  • Si la personne retenue est dittiste elle doit tre diplme dun programme de stage en dittique reconnu par Les dittistes du Canada.

  • Must be able to speak read and write French at the predetermined linguistic level.
  • Knowledge and experience with computers required.
  • Effective oral and written communication skills.
