POSITION SUMMARY

The Director of Resident Care - Clinical is primarily responsible to oversee the planning and co-ordination of resident care and providing management of nursing and personal care services including infection control to ensure an outstanding level of resident care is maintained and enhanced on a continuous basis in accordance with the policies of the County and applicable legislation. As a valued member of Long-Term Care and Seniors Services the Director of Resident Care - Clinical demonstrates a commitment to resident safety by providing quality care in accordance with the organizations mission vision and values. Duties within the Administrative Director of Care position may be required as needed.

DUTIES AND RESPONSIBILITIES (not listed in order of priority)

1. Commitment to and communication of Resident Care Mission including vision and annual goals and objectives for residents care and nursing services which are congruent with the mission statement of the Home.

2. Accountable in the development and coordination of a comprehensive needs based plan of care developed through such venues as admission annual and special care conferences resident and family meetings and ongoing consultation.

3. Monitor and maintain an organized program of nursing services to meet residents nursing and personal care needs consistent with the professional standards of practice of the College of Nurses and the Ministry of Health Long-Term Care Program Standards.

4. Maintains a wellness perspective and supports residents in achieving quality experiences. Respects and uses sensitivity in spiritual cultural and ethnic aspects of a residents life.

5. Facilitates relationships between and among residents relatives staff and community. Monitors resident family and community demand for services.

6. Seeks opportunities for resident and family (SDM/POA) participation in decisions that affect the residents rights and life style.

7. Monitors and improves attitudes and behaviours of family and staff to ensure inclusion of residents in decisions regarding their plan of care.

8. Creates opportunities for individuals to make choices and to take actions that will enhance the residents sense of mastery.

9. Creates a comfortable and safe environment that provides opportunities for privacy dignity control and maintenance of personal lifestyle.

10. Supports and helps provide a resident advocacy program though the promotion and respect of the Bill of Rights.

11. Communicates with and supports families in their involvement and participation. Promote resident and family centered care consistent with advances in gerontology.

12. Ensures an environment with ambiance conductive to the provision and practice of therapeutically effective resident health care yet be unobtrusive.

13. Collaborate with administration medical staff and other health care disciplines and services to meet the residents health care needs.

14. Provide a quality management program to continuously improve nursing and resident care services.

15. Participate and support nursing research.

16. Accountable for the admission discharge and transfer processes including approval of all Resident applications for admission to the Home and delegate as necessary. Liaise with Placement Coordination Service of the Community Care Access Centre and the Local Health Integrated Network.

17. Accountable for the delegation of clinical and supervisory responsibilities to appropriately qualified staff according to the applicable legislation and standards of the College of Nurses of Ontario and accountable for associated staff qualifications and credentialing requirements.

18. Monitor and promote standardized Quality Management program for the nursing department that is outcome-focused includes both quality improvement and risk management activities. Liaise with Quality and Development staff on audit results relating to charts restraints pain management wound care meal service/nourishments.

19. Accountable for the nursing department annual education plan in collaboration with the Nurse Manager and Program and Support Services Manager based on current clinical needs and mandatory requirements of the Ministry of Health Long-Term Care Program Standards and the County of Simcoe.

20. Oversee the pharmacological components of care including direct liaison with the contracted pharmacy to ensure excellent service provision quality assurance activities and ongoing education. Oversee medication administration reviewing reports on errors and audits. Accountable for all drug destructions and narcotics inventory counts following up on discrepancies with a summary analysis and action plan.

21. Provide input to the appointment of a Medical Director.

22. Liaise with the Medical Director to promote effective medical management of all residents that is resident-centered and meets all regulations and applicable legislation set by the Ontario Ministry of Health and Long-Term Care.

23. Ensure that information obtained by nursing staff is documented appropriately and is available to relevant members of the health care team resident and family in a professional manner consistent with facility policies and procedures for maintaining confidentiality of information consistent with the Municipal Freedom of Information and Protection of Privacy Act CNO guidelines/standards or other related and applicable legislation.

24. Delegation of clinical and supervisory responsibilities to appropriately qualified staff according to the applicable legislation and standards of the College of Nurses of Ontario.

25. Monitor a facility-wide infection control program in consultation with the IPAC Specialist to provide infection surveillance and outbreak management activities.

26. Develop and maintain effective relationships with the residents families the general public management other service providers committees organizations institutions and others. Promotes co-operation of services/programs with community agencies and other health care providers.

27. Manage the development and implementation of Nursing programs and related policies and procedures.

28. Provide leadership and supervision to Nursing Department staff in a manner that motivates guides and directs employees to the realization of departmental goals and objectives. Maintain a work environment that promotes staff participation teamwork and positive labour relations.

29. Coordinate and facilitate the restorative care functions of the interdisciplinary care team.

30. Liaise with the interdisciplinary team in order to identify assess facilitate document and follow-up on restorative care programs and initiatives.

31. Participates in and demonstrates an understanding of resident safety principles and practices into all day-to-day activities. Follows all safe work practices and procedures and immediately communicates any activity or action which may constitute a risk to resident safety.

32. Assist in human resources planning as well as recruitment performance evaluations counseling discipline and termination of personnel.

33. Prepare and present written reports as required.

34. Participate in committees as required mandatory lead of IPAC and Pharmacy & Therapeutics meetings and committees and mandatory participation of all required programs as outlined by MLTC.

35. Assume responsibility of the facility in the absence of the Administrator.

36. Effectively utilizes all electronic systems provided

37. Perform other duties as assigned including redeployment in emergency situations.

38. Works in compliance with the Occupational Health & Safety Act and the Long-Term Care Homes Act and their regulations in performing duties in a safe manner and follows all County of Simcoe Health & Safety policies procedures and legislation.

39. Maintain confidentiality in accordance with the Municipal Freedom of Information and Protection of Privacy Act and the Personal Health Information Protection Act.

EDUCATION TECHNICAL SKILLS AND CERTIFICATION

Strong knowledge of health care principles obtained through the completion of a degree in Nursing. A certificate obtained from a Gerontology related program preferred.

Current registration with the Ontario College of Nurses as a Registered Nurse.

Strong understanding of policies and related legislation or initiatives and their significance and potential impact.

Very good leadership and supervisory skills and ability to interpret and apply employment policies and collective agreements.

Strong interpersonal and communication skills.

Strong planning and organizational skills.

Strong time management and multi-tasking skills.

Knowledge of word-processing and spreadsheet application skills.

Criminal Records Check with Vulnerable Sector Screening that is less than six (6) months old at time of hire.

As a condition of employment candidates are required to submit documented results of TB testing as per Public Health requirements.

Valid G class drivers license and access to reliable vehicle.

EXPERIENCE

A minimum of five (5) years experience in progressively responsible related positions including at least one (1) year as a Registered Nurse in a Long-Term Care setting and at least three (3) years experience as a Registered Nurse in a managerial or supervisory role in a health care setting.

A minimum of two (2) years as a Director of Resident Care or similar position preferred.

Experience in the supervision of staff and the administration of employment policies and collective agreements.

EFFORT

Visual and analytical skills required when preparing reports problem solving etc.

WORKING CONDITIONS

This position is located in a long-term care facility exposure to illnesses and germs are possible on a daily basis.

Ability to work remotely if required.
