As a Perinatal Public Health Nursewithin East Central Public Health District you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.

Identify high risk populations and tailor interventions based on the needs of the population and resources available.
Provide home visiting services to identify assess and provide care coordination services to eligible perinatal patients and qualifying infants.
Coordinates program services with other service providers and community organizations to promote optimal implementation.
Screen and refer clients to other community service providers.
Monitoring per program guidelines educational instruction materials to promote wellness safety and pregnancy and postpartum complications.
Monitor for infant complications to include feeding weight head circumference and developmental screening.
Develop a plan of care for individuals and families modify treatment plans as indicated by clients responses and conditions.
Provide referral/linkage when medical and environmental risks are identified to appropriate services. Promote program through community engagement and outreach events.
Develop and sustain community partnerships
and Jefferson County.
Bachelors degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR associate degree in nursing from an accredited college or university AND One year of nursing experience AND current Georgia Registered Professional Nurse license AND CPR certification.
This position is considered a Category 4 under the pre-employment fitness-for duty

Current State employees are subject to State Personnel Board rules regarding salary.
DPH accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered. DPH will contact educational institutions to verify degree diploma licensure etc.
