GENERAL DESCRIPTION
Engages people supported and their families in the development of resourceful plans that assure the needs and wishes are addressed and life opportunities expanded. This role assists in the choice coordination and provision of services and supports while monitoring the delivery of those services and supports. Support Coordination promotes individuality choice and control while consistently reinforcing the values principals and philosophy of West Bay RI along with all applicable policies and procedures.
It is essential to possess a strong foundation of experience in providing support coordination or services to people with intellectual and developmental disabilities with a strong belief that people with disabilities can achieve life outcomes have value and contribute to their community. All decisions and actions will be consistent with the agency mission and will be completed with a person-centered service focus. Proficiency with technology computers and with RI State licensing requirements is essential.
RESPONSIBILITIES
Completes assessments to identify each persons needs and desires. This includes the use of formal and informal tools to gather information needed to assist the person to identify their needs and wishes. Assists the person supported to develop their Individual Support Plan (ISP) and personal budget with a person-centered approach. Updating the ISP as needed and at least quarterly. Identifies the services that will meet the individuals needs. Completes the referrals. Coordinates the services the person is receiving. Resolves any barriers to the delivery of service. This involves understanding a full array of options and knowing where to learn of others as needs and interest warrant. Supports families with concerns around service delivery goals and ISP development. Monitors the services through collaboration with other professionals to assure they are delivered as planned and that the outcomes that were identified are being achieved. Participates in weekly team meetings staff meetings and professional meetings. Assure a regular weekly presence in the homes to support the day-to-day organization within the home environment. Identifies and sustains the persons support network including family friends and associates. Develops a professional working relationship with the families and individuals to maintain a person-centered focus in their caseload. This involves getting to know each person we support.

Hours: 30 hours full time (Flexibility required)
Pay/Salary: $43680 ($28/hourly)
