Senior Team Counselor - Adult Community Clinical Services (ACCS)

ACCS Integrated Teams provide clinical interventions and peer and family support to facilitate engagement support functioning and maximize symptom stabilization and self-management of individuals and to assist individuals in developing skills establishing natural supports and resources to live successfully in the community.
The Senior Integrated Team Counselor will work as a member of a Persons Team having primary responsibility in delivering interventions identified in the treatment plan with clinical guidance from the Integrated Team clinical staff. This position will have a specialty role in at least one area of expertise which may include: building new relationships with community resources research and connection with various benefits and resources employment and education or representative payee.
The Senior Team Counselor will also be responsible for:
- Provide input to person-centered assessment and service planning and advocacy coordination and linkage with other services and the community.
- Model and teach skills for development of positive relationships and negotiation of conflict.
- Assist persons served in understanding managing and dispensing medications consistent with MAP and agency policies and procedures.
- Maintain regular daily monthly and annual clinical records in keeping with DMH agency and division policies and procedures and within required timelines.
Schedule: Full Time 40 hours
Pay Rate: $22.67/hourly

- Bachelors Degree and three years related experience required
