Main Purpose of Post
The main purpose of this role is to facilitate positive change in individuals lives through the application of social work methods models and tools. The primary objectives include promoting independence and managing risks of abuse and neglect within the framework of relevant legal and policy guidelines.
Key Responsibility Areas
- Manage a caseload of individuals with care and support needs and carers requiring support.
- Conduct assessments of care and support needs in accordance with the Care Act 2014 and associated regulations.
- Collaborate with individuals to develop care and support plans ensuring eligibility and local authority provisions are identified.
- Perform financial assessments related to care and support services.
- Provide information on service costs welfare rights entitlements and other relevant advice to individuals with care and support needs and carers.
- Arrange services provided or facilitated by the local authority as per care and support plans.
- Assess and arrange provision of simple equipment assistive technology and minor adaptations for individuals with care and support needs.
- Conduct reviews of care and support plans and support plans for carers.
- Reassess the needs of individuals with care and support needs and carers.
- Recognize and address safeguarding concerns for children and adults with care and support needs.
- Undertake safeguarding adults enquiries and develop safeguarding plans in line with relevant policies and procedures.
- Apply the Mental Capacity Act 2005 when necessary including assessing capacity for decision making and making best interest decisions.
- Attend court as a witness when required.
- Make recommendations for legal action when appropriate.
- Engage individuals with care and support needs and their carers in the decision-making process and ensure access to necessary support services.
- Maintain accurate record-keeping in compliance with organizational requirements.
- Identify unmet needs due to resource limitations and report to the manager.
- Collate information required for performance and quality monitoring.
- Contribute to the investigation and resolution of complaints and inquiries.
- Address immediate situations when serious incidents occur and report to the manager.
- Prepare and present reports and assessments for meetings reviews and legal proceedings.
- Keep skills and knowledge up-to-date in compliance with professional registration requirements.
- Work flexibly and participate in interdisciplinary groups and task teams.
- Take responsibility for addressing racism and promoting good race ethnic and community relations.
