The community access direct support staff program lead will assist individuals while in the community and are required to follow the policies procedures and standards of care while performing the various responsibilities:
- Monitoring and Supervising all Community Access Direct Support Staff
- Executing the tasks and goals of the ISP.
- Monitoring and Review of Behavior and ISP tracking pulling and creating detailed reports.
- Planning Organizing and Executing planned activity calendar effectively.
- Supporting the individual with increasing their independence in the community.
- Attending and contributing to team meetings.
- Assisting in transporting the individuals to various community activities in accordance to the community activity schedule.
- Assisting with health and wellness related tasks (if required by the ISP).
- Scheduling and planning meaningful community activities with individuals.
- Providing advice and guidance to assist with decision-making and serving as an advocate to encourage the individual.
- Assisting with documentation all activities and progress of the individuals on a daily basis.
- Completing documentation and goal tracking in a timely manner as needed
- Working as a team with other employees to provide the best quality of support for the individual.
- Utilizing proper behavior support training and crisis prevention intervention techniques when necessary.
- Working well in a self supervisory setting which includes taking initiative when necessary and engaging with individuals.
Qualifications :
- Bachelors or Associate Degree in ( Psychology Social WorkHuman Services Education Public Health or any Health/ Social/ or Human Services related degree) preferred or High School Diploma/ GED with at least 5 years supervisory experience in Adult/ Child Day SupportDay Care Home Health Careor Nursing Home Activities.
- First Aid and CPR certification
- A minimum of two (2) years of recent experience in related field day program services home health care or working with individuals with developmental disabilities autism and any other intellectual disabilities.
- Above average communication skills: orally and written
- Must be able to write/ type documentation
- Preferably familiar with Therap or any other virtual documentation system
- Must be able to work independently
- Must be familiar with working with behaviors and know how to apply redirection and or crisis prevention techniques.
- Must be creative and willing to think outside the box
- Must have the ability to utilize effective decision making
- Must be able to work independently without direct supervision
- Must posses strong organizational and leadership skills
- Must be willing to follow ALL COVID-19 guidelines at all times in accordance to the recommendations from the CDC and the policies of GA DBHDD
