Hospice After-Hours Registered Nurse
(7 Days On / 7 Days Off)

Your Impact as an After-Hours RN
As an After-Hours Registered Nurse you will be responsible for assessing coordinating and delivering patient care during evenings nights and weekends. Your role includes:
Performing initial assessments and follow-up evaluations for a home health patient caseload.
Identifying physical emotional and environmental care needs and developing safety-focused care plans.
Communicating critical updates to the clinical supervisor physicians and care teams.
Responding to changes in patient condition and initiating appropriate nursing interventions.
Implementing and verifying physician orders in line with individualized care plans.
Maintaining accurate and timely clinical documentation.
Participating in case conferences and collaborating to ensure continuity and quality of care.
Performing other duties and special projects as assigned.
What Youll Need to Succeed
Graduate of an accredited nursing program (Certificate Diploma Associate or Bachelors)
Current RN license in the state of practice
At least 1 year of clinical nursing experience (home health preferred)
Willingness and ability to travel approximately 50%
Strong critical thinking communication and documentation skills
Compassion reliability and a patient-first mindset
