Hospice After-Hours Registered Nurse

(7 Days On / 7 Days Off)

Your Impact as an After-Hours RN

As an After-Hours Registered Nurse you will be responsible for assessing coordinating and delivering patient care during evenings nights and weekends. Your role includes:

  • Performing initial assessments and follow-up evaluations for a home health patient caseload.

  • Identifying physical emotional and environmental care needs and developing safety-focused care plans.

  • Communicating critical updates to the clinical supervisor physicians and care teams.

  • Responding to changes in patient condition and initiating appropriate nursing interventions.

  • Implementing and verifying physician orders in line with individualized care plans.

  • Maintaining accurate and timely clinical documentation.

  • Participating in case conferences and collaborating to ensure continuity and quality of care.

  • Performing other duties and special projects as assigned.

What Youll Need to Succeed

  • Graduate of an accredited nursing program (Certificate Diploma Associate or Bachelors)

  • Current RN license in the state of practice

  • At least 1 year of clinical nursing experience (home health preferred)

  • Willingness and ability to travel approximately 50%

  • Strong critical thinking communication and documentation skills

  • Compassion reliability and a patient-first mindset
