Youll focus on aged care assessment and planning through online meetings and phone calls with clients and their support networks not hands-on care or treatment making it ideal for nurses ready to grow in a strategic structured environment.

What youll do
- Conduct personalised assessments for new and existing Home Care Package clients
- Translate needs into clear client-focused care plans using enablement frameworks
- Recommend services and supports tailored to individual lifestyles and goals
- Document and update assessments using digital care systems (well train you)
- Work collaboratively with your team clients and families
What were looking for
- Enrolled Nurses Registered Nurses or professionals with Cert III in Individual Support Aged Care
- Confident communicators with empathy emotional intelligence and strong decision-making skills
- Experience with aged care HCP NDIS or care planning (ideal but not essential)
- Digital confidence with Microsoft 365 Outlook Teams and CRMs
- People who want to grow learn and build a sustainable career in care
