What Youll Do in This Role:
In this role the Certified Nurse Aide (CNA) provides direct hands-on care to participants aged 55 and older in an adult day center setting in alignment with PACE (Program of All-Inclusive Care for the Elderly) guidelines. The CNA plays a vital role in promoting independence dignity and overall quality of life by delivering compassionate person-centered support tailored to each participants unique needs.
Primary Responsibilities:
Participant Care
- Provide personal care homemaking and respite support in participants homes and the day center as needed.
- Assist with activities of daily living (ADLs) per individualized care plans including bathing dressing grooming toileting feeding and mobility support.
Collaboration
- Partner with members of the Interdisciplinary Team (IDT) to ensure participant care needs are addressed.
- Effectively communicate observations updates and concerns with team members to promote continuity of care.
Documentation
- Accurately record all services provided and participant observations in compliance with organizational policies and regulatory standards.
Environment Support
- Maintain a safe and clean environment in both home and day center settings.
- Assist with light housekeeping and meal preparation as needed.
Other Duties
- Participate in ongoing training and team meetings to stay current on PACE procedures and protocols.
- Support activities that enhance participants independence dignity and quality of life.
Skills & Knowledge
- Physical Capability: Able to assist with participant transfers mobility and all activities of daily living (ADLs).
- Communication: Excellent verbal and written communication skills; able to clearly explain tasks listen attentively and engage effectively with older adults and their families.
- Professionalism & Compassion: Demonstrates empathy integrity and high ethical standards in all interactions with participants families and interdisciplinary team members.
- Teamwork & Independence: Works well both independently and collaboratively; flexible and dependable in a fast-paced changing environment.
- Problem Solving & Adaptability: Strong critical thinking and problem-solving skills with the ability to prioritize and creatively adjust to evolving care needs.
- Technical Proficiency: Proficient in Microsoft Office Suite (Excel Outlook Word) and experienced with electronic medical record (EMR) systems.
What Youll Need:
- Certified Nurse Aide (CNA): Current certification in the State of Michigan from an accredited program.
- Education: High school diploma or equivalent preferred (may not be required for all candidates).
- CPR Certification: Current certification or willingness to obtain within the first 30 days of employment.
- Transportation: Reliable transportation and a valid drivers license required for travel within the service area.
Experience
- Minimum of one year of experience working with the frail elderly in long-term care or with individuals receiving end-of-life care is preferred.
