Level 3 Support Coordinator
Northumberland - Blyth and surrounding areas
Permanent full time (37.5 hpw Nightshifts
Salary from 25838 to 26645 per annum depending on skills and experience plusgreat benefits including Health Cash Plan

Typical day as a Level 3 Support Coordinator
- Creating support plans with your customers and coordinating the shift.
- Helping customers who have complex mental health learning disabilities and dual diagnosis with daily living activities such as making meals shopping budgeting maintaining their tenancy and moving onto independent accommodation if they are able.
- Carrying out risk assessments support planning goal setting and regular reviews.
- Cuppas chats and catch ups. Theres no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring
- Passion to support our customers to live their best life working collaboratively with an eye for detail.
- Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
- Experience of coordinating and assessing customer referrals.
- The ability to workon your own initiative remaincalm under pressure and havea resilient approach
- To get from A to B youll need a vehicle insured for business purposes. The great news is that well pay your mileage!

Job details
- Where were flexible with you we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
- Working 8pm until 8am on a 3 on and 3 off working pattern.
- Although youll work nights we do get together regularly in the daytime for training and team meetings.
- Able to use technology for creating and updating support plans complete online learning and to collaborate with colleagues.
- Youll need an Enhanced DBS check done and we pay for that.
