With over 400 employees in Southern Colorado you would be joining a mighty team of support and administrative staff clinicians physicians nurses and others in our efforts to improve the health and well-being of our community.

Were hiring for a full-time Clinician to join our Community Response/SOS Team 41 Montebello Rd. Pueblo CO 81001.

Work schedule: 12-hour shifts

10 am 10 pm varied Monday through Saturday

Report to Supervisor of team

What Youll Be Doing

As the Clinician for the Solutions Outreach Services (SOS) Team you will provide mobile community-based services in response to calls identified by Pueblo Police Dispatch. These calls involve behavioral health concerns that do not require law enforcement intervention. In this role you will conduct behavioral health outreach to include but not limited to clinical assessments de-escalation case management and facilitate connections to appropriate behavioral health services.

Your work is essential to reducing unnecessary law enforcement involvement and ensuring individuals receive timely and appropriate care. Additionally you will support and educate family members equipping them with the information and resources needed to assist loved ones experiencing mental illness and/or substance use disorders as well as to care for their own well-being.

Key Responsibilities:

  • Respond to behavioral health-related calls dispatched by the Pueblo Police Department that do not require a law enforcement officer on-site.
  • Triage and assess clients in the field determining risk need and eligibility for services through Health Solutions and other community resources.
  • Deliver services in the community or home settings depending on client needs.
  • Offer case management support including service linkage follow-up crisis planning and basic needs assistance (e.g. housing food benefits medical and legal referrals).
  • Complete necessary documentation such as the VI-SPDAT and input encounters into HMIS (Homeless Management Information System).
  • Assist in homeless outreach efforts in collaboration with street outreach teams and the Pueblo Police Department.
  • Act as a liaison and make referrals to community agencies service providers and natural support systems to facilitate integrated care.
  • Collaborate with individuals families and significant others in treatment planning and decision-making.
  • Educate and support families to increase understanding of mental health substance use disorders and the recovery process.
  • Coordinate with internal Health Solutions programs including Crisis Services Case Management Care Coordination Outpatient and Recovery Services to ensure continuity of care.
  • Participate in interagency collaboration with law enforcement jail diversion programs the courts and other criminal justice entities.
  • Provide consultation to law enforcement officers and dispatch on crisis de-escalation and behavioral health awareness.
  • Utilize evidence-based practices and comply with Health Solutions policies state standards and clinical guidelines.
  • Maintain timely accurate documentation and adhere to all data reporting requirements.
  • Attend clinical consultations team meetings and staffings as required.
  • Assist management with program reporting presentations and data tracking as assigned.
  • This is a field-based role not office-based employees will work out of a fully equipped company van and respond to service calls throughout the day.
  • This is a driving-intensive position that requires safe and reliable operation of a company vehicle throughout your shift.

Physical requirements include the following:

  • Requires full range of body motion.
  • Manual and finger dexterity and eye-hand coordination.
  • Requires standing walking and staying stationary for prolonged periods.
  • Occasionally lifts and carries items up to 25 pounds.
  • Requires exposure to communicable diseases and bodily fluids.
  • Requires working under stress in emergencies.
  • Requires a normal range of hearing and eyesight to record data electronically and communicate with patients.
  • Frequently operatesa computer and other office equipment to record health data.
  • Constantly communicates with clients and others.
  • Frequently drives between facilities or for other assigned duties.

Qualifications

What Were Looking ForThe Must-Haves

  • Masters degree in social work counseling psychology or a closely related field (required); candidates must be eligible for licensure within a defined time frame.
  • Experience working with substance use disorders (SUD) and co-occurring disorders (required).
  • Proficiency in conducting brief therapy and working with diverse populations including various age groups and individuals experiencing acute behavioral health crises.
  • Strong computer literacy and ability to use electronic health records and other clinical systems.
  • Valid Colorado drivers license
  • Knowledge of behavioral health systems trauma-informed care harm reduction and motivational interviewing.
  • Strong interpersonal organizational and communication skills.
  • Comfortable working in field-based and fast-paced environments including homes streets and public spaces and with law enforcement.

What Wed Like to See in YouThe Nice-to-Haves

  • Colorado clinical license (i.e. LPC LCSW LMFT)
  • Spanish language skills
  • Colorado CAC II III or LAC
  • Experience in community-based crisis intervention case management or working with law enforcement is strongly preferred.
