Tricare Home Health Services Inc. a trusted family-owned agency serving San Antonio since 1996 is seeking a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to join our close-knit office team. This full-time in-office role is ideal for someone who enjoys wearing many hats solving problems on the fly and playing a key role in keeping our agency running smoothly.
This isnt a typical desk job youll help with everything from chart reviews and care coordination to compliance and patient follow-ups. If youre proactive organized and excited to be part of a team where your input truly matters we want to hear from you.
What Youll Be Doing:
- Care Coordination & Support Assist with patient records care plan updates and survey prep
- Patient & Provider Communication Follow up on orders coordinate with doctors and help patients get the services they need
- Compliance & Quality Assurance Help ensure documentation is accurate and that were always ready for audits or visits
- Jump In Where Needed Support intake handle clinical questions assist the administrator or DON be the reliable go-to

Who Were Looking For:
- Licensed LVN or RN in Texas
- 13 years of nursing experience (home health experience preferred but not required)
- Independent & Adaptable Youre comfortable working without constant supervision and love variety
- Excellent Communicator You work well with patients doctors and teammates
- Detail-Oriented Youre great at spotting whats missing and making sure nothing falls through the cracks
- Spanish-speaking is a plus but not required
