About the Role:
The Supportive Housing Case Manager will work with a caseload of 10-20 single and family households in supportive housing. The clients in these programs experience a number of concerns from mental health diagnoses substance abuse physical/developmental disabilities child protection involvement limited income domestic violence and chronic or long-term homelessness. He/she will support the housing first model with a trauma informed care approach that will maximize the likelihood of client success and long-term self-sufficiency. Responsibilities include: conducting individual needs assessments program planning empowering clients to maintain their housing and ultimately empower client to work toward self-sufficiency. The Case Manager will guide both single and family households to services and resources to help them stabilize rebuild their lives and graduate out of the Supportive Housing program.
