Position Overview:
The Director House Calls will be responsible for overseeing the clinical and operational functions of the provider team ensuring the delivery of high-quality patient-centered care. This leadership role involves
managing provider scheduling facilitating training and professional development coordinating cross-coverage and serving as a clinical resource for complex case management. This role plays a critical role in
advancing the organizations mission to deliver exceptional in-home medical services.
Responsibilities:

- Supervise and manage a team of Advanced Practice Nurses Physician Assistants and support staff providing in-home patient care.
- Oversee daily operations including provider scheduling coverage planning and performance management.
- Provide clinical leadership offering support consultation and mentorship to the provider team.
- Coordinate and deliver training programs for clinical and operational competencies.
- Monitor clinical performance to ensure compliance with organizational standards best practices and regulatory requirements.
- Collaborate with administrative staff to optimize workflows patient scheduling and provider deployment.
- Serve as a clinical resource for complex case management patient care protocols and escalation of clinical concerns.
- Overseeing the clinical and operational functions of the provider team ensuring the delivery of high quality patient-centered care.
- Participate in recruitment onboarding and ongoing education of new providers.
- Work closely with the leadership to implement strategic initiatives quality improvement projects and patient care standards.
- Conduct regular team meetings and one-on-one check-ins with providers to foster engagement professional development and high morale.
- Demonstrate excellent guest service to internal team members and patients.
- Perform other related duties as assigned.
Qualifications:
- Advanced Practice Registered Nurse (APRN) degree (Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)).
- Current licensure as an autonomous Advanced Practice Registered Nurse (APRN) in the state of Florida.
- Certification as a Nurse Practitioner (NP) preferably in Family Adult-Gerontology or a similar specialty.
- 5 years of clinical experience prior clinical management experience preferred.
- Experience in home-based care primary care or chronic disease management is strongly preferred.
- Demonstrated excellence in clinical practice and operational management.
- Ability to work collaboratively with interdisciplinary teams and leadership.
- Ability to travel regionally within Florida and/or assigned regions as needed.
- Possess strong clinical expertise proven leadership abilities and a commitment to excellence in home based healthcare.
- Strong interpersonal communication and leadership skills.
- Ability to serve as a proactive leader committed to excellence in clinical practice and team development.
- Ability to work independently in a fast-paced cross-functional environment.
Physical Demands
- Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift carry push pull or otherwise move objects. Repetitive motion. Substantial movements (motions) of the wrists hands and/or fingers. The worker must have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading. Ability to lift up to 15 lbs. independently not to exceed 50 lbs. without help.
