Job Description

The RN Clinical Coordinator is responsible for care coordination and supporting clinical quality across health center sites. This position plays a central role in case management functions including hospital transfers discharge follow-up coordination of care with primary care providers and monitoring of clinical quality measures. The RN Clinical Coordinator supports the standardization of clinical processes and assists with evaluating the skills and competencies of clinical staff. This position also participates in quality improvement regulatory compliance immunization oversight and clinical triage as needed.

KEY ACCOUNTABILITIES:

  1. Coordinate and oversee case management and care coordination activities.
  2. Monitor and support clinical quality measures and continuous improvement initiatives.
  3. Assist in evaluating clinical staff skills and competencies.
  4. Provide oversight and support for immunization programs across multiple sites.
  5. Provide clinical triage support and contribute to patient care when needed.
  6. Collaborate across departments to support integrated patient-centered care delivery.
  7. Support the organization as needed.

KNOWLEGE SKILLS AND ABILITIES:

  1. Ability to provide a positive team-oriented environment.

  3. Knowledge of primary care nursing practice OB pediatrics chronic care conditions and immunizations.
  4. Knowledge of Patient-Centered Medical Home (PCMH) model of care delivery Federally Qualified Health Center (FQHC) and quality metrics such as UDS GPRA and HEDIS.
  5. Understanding of accreditation requirements.
  6. Proficient in case management.
  7. Effective verbal and written communication skills.
  8. Culturally competent; able to work with a diverse workforce and client population.
  9. Open to change and willing to lead change within the organization.
  10. Professional integrity; excellent work ethic.
  11. Able to manage competing priorities; excellent organizational skills and enjoyable time management.
  12. Ability to work independently and take initiative in addressing concerns and issues.
  13. Proficient with computer-based applications such as MS Office Adobe registries/medical databases and electronic health records.
  14. Knowledge of community resources.
  15. Must have a sincere desire to improve the health status of minority and vulnerable populations as well as being willing to serve the uninsured and underinsured.

Qualifications :

  1. A bachelors degree from an accredited school or college of nursing is required.
  2. Licensed to practice nursing in the state of Arizona as RN is required.
  3. At least three (3) years nursing experience.

Standard Employment Requirements: 

  1. Must possess a valid and unrestricted Arizona drivers license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona drivers license during employment. Must be insurable under NATIVE HEALTHs liability auto policy.
  2. Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
  3. Must pass substance abuse testing upon employment and submit to random testing during employment.
  4. Must provide an updated immunization (IZ) record at the time of employment.
  5. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
  6. Must obtain Basic Life Support (BLS) certification within (90) calendar days of employment and maintain certification during employment.
