Job Description
Location: Columbia Tennessee 38401 (Hybrid)
Start Date: July 1 2025
End Date: June 30 2026
Work Schedule: Monday Friday 8:00 AM 4:30 PM
Pay Rate: Up to $27.23/hour
Key Responsibilities:
- Social Support Navigation (SDOH):
- Maintain a resource list for food housing mental health and other community services.
- Collaborate with nurses and providers to identify and address patients social needs.
- Conduct in-person or remote screenings for patients referred by primary care teams.
- Refer and connect patients to community resources and services.
- Case Management:
- Support hospital discharge planning and care coordination.
- Help obtain medical records and needed medical equipment.
- Conduct follow-up on care plans and assist patients in returning to care.
- Specialty Referral Navigation:
- Coordinate and track specialist and imaging referrals.
- Obtain records from referral providers.
- Maintain a resource list for specialty providers including eligibility and cost info.
- Patient Assistance & Medication Access:
- Help patients find and apply for low-cost prescription programs.
- Assist with applications for programs such as CoverRx and RxOutreach.
- Act as a liaison between patients and pharmaceutical companies when needed.
- Other Duties:
- Participate in clinical team meetings and community outreach events.
- Document activities in the patient s medical record and consult with healthcare staff.
- Provide patient education or connect patients with appropriate educational resources.
Minimum Qualifications:
- Education: Bachelors degree in Social Work or another related field.
- Experience: Minimum of 1 year in a relevant role.
