Description of Job

The Occurrence Reporter coordinates organization-wide efforts to ensure that performance management and quality improvement initiatives are developed managed implemented and maintained utilizing data-driven Lean-focused techniques in concert with the CMHHIFL Strategic Plan. Hospital-wide the Occurrence Reporter helps to foster a performance-based culture that continually evolves and improves.

CMHHIFL Incident Tracking Monitoring and Reporting:
Responsible for hospital-wide critical incident tracking monitoring and reporting programs utilizing the hospitals RL Datix incident reporting system.

Quality Assurance
Initiates performance improvement projects based on incident trends and areas identified for improvement utilizing the Plan Do Study Act or Lean models.
  • Assists in the implementation of the Quality Management process that leads to positive and measurable impacts of CMHHIFLs strategic Quality Assurance and Performance Improvement Plan.
  • Coordinates performance management and quality improvement projects throughout the hospital.
  • Designs develops and implements organization PI initiatives thereby positively impacting patient care across all disciplines.
  • Assists other disciplines with writing and revising hospital policies and serves as a quality management representative on policy-writing workgroups and committees. Ensures hospital policies align with regulatory standards.
Data Analysis Collection and Dashboards
Analyzes data and develops reports for dissemination to the Colorado Mental Health Hospital in Fort Logan Executive Committee Quality Council OCFMH executive leadership and other stakeholders.
  • Identifies trends and implements strategic performance improvement projects as a result of identified gap areas.
  • Collaborates with the RL Datix team quality department and hospital subject matter experts on the design of the information technology infrastructure required to support a statewide performance and quality improvement system.
  • Ensures needed data is collected on a timely basis regular reports on progress are distributed and makes recommendations for further improvements based on the data.
  • Provides monthly data analysis with regard to hospital incidents performance improvement projects and standing committees aimed at meeting regulatory standards.
  • Tracks data to improve operational and clinical performance and promote accountability and transparency.
  • Collaborates regularly with the leadership groups of the hospital including MEC Nurse Managers medical staff and line staff as it relates to incidents plans of action and reporting of occurrences to assist hospital in following regulatory standards
  • Develops and manages dashboard to determine program viability identify areas for improvement make changes and reallocate resources if necessary
Regulatory Compliance
Serve as a resource hospital-wide for standards regulations compliance trends and propose initiatives by providing training consultation conceptual framework and structure to hospital staff.
  • Work with hospital management and staff to determine the level of compliance with regulatory standards and rules and develop plans for compliance.
  • Work with the Joint Commission and other accrediting and regulatory bodies to report and correct problems. Monitor various publications websites and other media for current standard changes and issues.
  • Determines and implements the tracer process including system and individual tracers.
  • During survey years leads and develops preparatory activities including training and auditing of treatment units for compliance with standards.
  • Develops and submits Evidence of Standard Compliance documentation to The Joint Commission
  • Works with hospital management and staff to determine the level of compliance with regulatory standards and rules and develops plans for compliance.
  • Works with the Joint Commission and other accrediting and regulatory bodies to report and correct problems.
  • During survey years leads and develops preparatory activities including training and auditing of treatment units for compliance with standards.
  • Develops and submits Evidence of Standard Compliance documentation to The Joint Commission.
Collaboration and Training
Participates in Quality Management Department meetings.
  • Provides consultation and guidance to hospital staff with regard to quality work and patient safety.
  • Collaborates with external entities with regard to on-going performance improvement on behalf of CMHHIFL. May require travel within the State to meetings with regulatory partners or other CDHS partners.
  • Collaborates with staff development to revise existing training materials and create new ones developing training based on quality issues found as part of tracers and incident management.

MINIMUM QUALIFICATIONS:

Experience Only:

Six (6) years of relevant experience in an occupation related to the work assigned to this position

OR
Education and Experience:

A combination of related education and/or relevant experience in an occupation related to the work assigned to this position equal to six (6) years


Preferred Qualifications:
  • Bachelors degree or higher in the clinical field of Counseling Psychology Nursing or Social Work.
  • Five or more (5) years of experience working in a clinical or hospital setting.
  • Behavioral health experience
  • Experience investigating occurrences
  • Prior utilization management experience
  • Experience in writing professional reports
  • Prior experience interacting with regulatory entities
  • Previous experience in quality improvement measures
  • Data collection and analysis
  • Experience with Google Suite Applications
