REPORTS TO: Sr. Administrator of Nursing

WORK SCHEDULE: 188 days / 7.5 hrs. - Union UFCW 1459 contract

BRIEF JOB DESCRIPTION Case Management is a collaborative process of assessment planning facilitation care coordination evaluation and advocacy for options and services to meet an individuals and familys comprehensive health needs. The School Nurse Case manager will work in collaboration and continuous partnership with chronically ill or high-risk students and their family/caregiver(s) local providers and nursing department staff to improved health outcomes and academic success for students and the district.

PRIMARY FUNCTIONS & KEY RESPONSIBILITIES OF THE JOB:

  • Serves as a school nurse educator float nurse and liaison between students families and school health services
  • Participates in direct and indirect nursing care health maintenance teaching counseling collaborative planning and rehabilitation.
  • Serve as a point of contact between school nurses staff family community and health care providers.
  • Proactively advocates for students and school nurses by responding to concerns with empathy and respect addressing needs concerns and preferences while identifying opportunities for improvement to support student health and academic success.
  • Participate as the health expert for Individualized Education Plan (IEP) and 504 plans. Promote adherence to developed plans of care. Aid in development of this IHP with the student family/caregiver(s) and providers (emergency plan health management plan and ongoing action plan as appropriate)
  • Serves as a float school nurse demonstrating expertise in health office operations district policies and protocols and school health laws to support coverage during nurse absences and peer school nurses as directed by nurse administration.
  • Responsible for school nurse orientation activities offering ongoing support and follow-up to ensure successful integration and performance of newly hired school nurses skills training and competency development.
  • Identify high-risk students (the chronically ill chronically absent (greater than 18days) those with special health care needs) through a collaborative approach to create a caseload for direct management.
    • Assess unmet health and social needs and evaluate medical history including social behavioral psychological and familial concerns.
    • Promote timely access to appropriate care.
    • Increase utilization of preventative and tertiary care
    • Reduce school dismissals and/or absences emergency room utilization
    • Increase continuity of care by managing relationships with care providers referrals and staff.
    • Connect students to relevant resources with the goal of enhancing student health and well-being
    • Foster school connectedness and academic achievement.
    • Plan for appropriate evidence-based interventions in the school setting
    • Incorporate the students goals and related activities into a student-centered plan of care. Evaluate and facilitate changes to revise plan(s) as necessary.
    • Monitor adherence to care plans 504s IEPs and the students progress in a timely manner
    • Monitor the application of school nursing care ensuring a high quality of care is being given
    • Create ongoing processes for patient and family/caregiver(s) to determine and request the level of care coordination support they desire at any given point in time
    • Evaluation of the outcomes
    • Attend/participate/facilitate home visits discharge meetings attendance of medical appointments and school-based meetings to carry out the coordination of plans
  • Participate in required CSHS leadership meetings and conduct yearly quality improvement (QI) initiatives for departmental improvement opportunities.
  • Proficient in Excel or similar workbook applications creating data tracking spreadsheet for data reporting and outcome evaluation.
  • Ability to prepare educate track and monitor monthly health office data reported by nursing staff to aid in identifying trends or areas of need.
  • Annually prepare and submit presentations and written reports on case management tasks and caseloads to nurse administration with additional reporting requirements outlined by Quality Improvement (QI) projects.
  • Provide additional tasks/assignments as delegated by nursing administration.
  • Meet routinely with nurse administration.

MINIMUM JOB QUALIFICATIONS:

  • Must be DOE certified or certifiable by the Department of Education in educator category Nurse All Levels. (DOE certification eligibility currently requires a minimum of a BSN.) Professional Licensure preferred.
  • RN with greater than 5-years school nursing experience preferred
  • Current CPR and First Aid certification
  • Current BLS Instructor preferred

PHYSICAL REQUIREMENTS:

  • No lifting bending or twisting restrictions.

WORKING ENVIRONMENT

  • This position requires the employee to be in an office or similar indoor environment. Requires ability to travel amongst Springfield public schools locations.
