Job Description

Position Summary
Flexible work arrangements including virtual/hybrid work locations are an integral part of the School of Medicines Working Forward initiative. As such this positions work location is designated as hybrid. Please note that the designated work location is subject to change based on the units business needs. This position will support the North Carolina Psychiatry Access Line (NC- PAL ) Intellectual and Developmental Disabilities ( IDD ) team a statewide psychiatric access program implemented in collaboration with North Carolina Department of Health and Human Services and Duke University. NC- PAL consists of professionals from multiple disciplines (psychiatry pediatrics nursing social work public health) who implement different facets of the program. Objectives of the IDD program include: 1) develop and implement a pilot training and service program with pediatric practices offering outreach and education via toolkits webinars case discussions and in-person training; 2) provide peer-led family support for guardians and families of children with IDD ; 3) increase knowledge and capacity related to addressing IDD for internal NC PAL staff. Our goal is to help clinical care teams provide more efficient effective and family-centered care that optimizes quality of life and well-being for children with IDD and their families. We are seeking to incorporate a team member with lived experience of raising and caring for a child with IDD into pilot primary care physician ( PCP ) offices. This position will serve as a professional peer resource for families seen by PCPs in the pilot program and will provide education training and guidance to pilot PCPs and practice staff on how to effectively support children with IDD and their families including connection to local community resources. Responsibilities of the family partner include the following: conduct needs assessment with patients and family to identify needed resources and communicate identified needs with practice staff build and provide tailored support for pilot practices provide training for practice staff on family navigation service systems and community resources initiate and respond to contact from families and community service providers receive and track referrals and outcomes of support provided demonstrate accurate clear professional and timely communication and engage in regularly scheduled team meetings peer support and supervision. This position is part time approximately 20 hours/week.

Required Qualifications Competencies And Experience
Have lived experience as a primary caregiver decision maker and navigation of the system for a child with IDD . Have or be willing to obtain Family Peer Specialist Certification. The family partner will be paired with a primary care physician in the Western region of the state and should live in or live in close proximity to Buncombe County.

Preferred Qualifications Competencies And Experience
Preferred skills include proficiency using Microsoft Word Microsoft Teams and Excel strong written and verbal communication skills Spanish speaking demonstration of personal and professional integrity and a willingness to be a part of a positive and collaborative work environment.
