Hourly Rate: $19.50

Duties & Responsibilities

  • The Lead Wraparound Facilitator will serve as a supportive role to Wraparound Facilitators. By providing individualized purpose-driven shadowing and training experiences to the Facilitator staff the hope of the Lead Facilitator role is to create an environment that promotes high fidelity to the wraparound model.
  • Provides services to a reduced caseload of no more than 5 families to allow for opportunities to work closely with facilitators who are identified as needing additional peer support.
  • Provides outreach and engagement to clients upon referral to the program by conducting an initial phone intake process with the family collecting basic information and presenting information to team to determine eligibility.
  • Coordinates with families and Assessors to aid in completion of CANS Assessments.

Qualifications

  • Bachelors degree from an accredited College or University in a social human services or behavioral sciences field required. Bachelors degree in other fields will be accepted with a minimum of two years of full-time experience working in relevant family children/youth or community service capacity. Masters Degree Preferred.
  • Must have two years of experience working as a Wraparound Facilitator.
  • Must be capable of extensive travel and driving. Must be able to provide 24-hour 7 day a week on-call crisis intervention to youth. Must be able to work a flexible schedule that includes some nights and weekends. Ability to transport youth volunteers and others.

About our Program:

Eckerd Connects Wraparound Agencies are part of the Coordinated System of Care (CSoC) under the states behavioral health managed care program that serves children and adults in Louisiana with behavioral health mental health and addictive disorders. Coordinated Systems of Care are national evidence-based models for coordinating services in a family-driven manner to ensure more positive outcomes for children youth and families. The model incorporates a broad array of flexible effective services and supports that is organized into a coordinated network and that integrates care planning and management across multiple levels and systems. Through this coordinated network meaningful partnerships are built with families and youth at service delivery management and policy levels. Family-driven and youth-guided services are provided in the home as well as in community settings that are data-driven and outcome oriented. Treatment plans are individualized and strength-based culturally and linguistically competent and integrated across all child-serving systems.
