Job Description
Position- Physical Therapist- Home Health
Facility- Providence Home Health LA County South
Address- 5315 Torrance Blvd Ste 169-B Torrance CA 90503
Area- LA County South and surrounding areas
Contract- 13 weeks (with a high chance of extention)
Shift- 08:00 - 16:30
Rate- $72-$75/hr
Start Date: ASAP
Shift: 8-hour Days (Facility does not accommodate 4x10s or 3x12s)
Patient Ratio: 1:1 Up to 6 patient visits/day (30 productivity points standard)
Experience Required: Minimum 1 year of skilled PT experience; home health experience preferred
First-Time Travelers: Accepted
Weekend Requirement: TBD
Certifications Required: BCLS
On-Call Requirement: None
Floating Requirement: None
Block Scheduling: Not available
Local Candidates: Accepted Same rate within 50-mile radius
Pending License: Accepted
RTO Restrictions: Must be confirmed with the hiring manager
Guaranteed Hours: Facility may cancel up to 1 shift per pay period (every 2 weeks)
