Job Description

As the Clinical Nurse Specialist Diabetes you will play a pivotal role in improving the health outcomes of Detainees across the Immigration Detention network by providing education support and evidence-based interventions for diabetes management. This role involves working closely with interdisciplinary teams developing personalised care plans teaching self-management skills and promoting a holistic understanding of diabetes to empower Detainees in managing their condition effectively.

Policy and Procedure Development: Develop and implement policies and procedures for diabetes management and education within the Immigration Detention facilities and as required Residence Determination (Community).

Diabetes Assessment and Education: Conduct comprehensive assessments to evaluate patients diabetes management including their medical history lifestyle cultural factors and psychosocial influences. Provide one-on-one and group education sessions to Detainees on diabetes self-management including medication adherence glucose monitoring insulin administration and nutritional guidelines.

Diabetes Management Support: Develop personalised diabetes care plans including meal planning medication schedules and physical activity recommendations. Work closely with dieticians endocrinologists and other healthcare professionals to ensure a multidisciplinary approach to diabetes management.

Health Promotion and Community Outreach: Develop and deliver workshops educational materials and support groups to promote diabetes awareness prevention and self-management strategies.

Collaboration and Interdisciplinary Teamwork: Collaborate with physicians nurses dietitians and other healthcare professionals to integrate diabetes education into comprehensive care plans.

About you:

  • Post graduate qualifications in Diabetes Management and Education
  • Minimum of a bachelors degree in nursing Nutrition or a related healthcare field.
  • Minimum 5 years of experience in a similar role in a clinical or community health care setting.
  • In-depth knowledge of diabetes management including current treatment protocols and technologies (e.g. insulin pumps continuous glucose monitors).
  • Excellent communication counselling and education skills with the ability to convey complex information in an accessible manner.
  • Ability to work with culturally diverse populations and adapt education to meet various health literacy levels.
  • Experience working in diabetes education and management particularly in settings serving diverse or underserved populations.
  • Demonstrated success in developing and implementing diabetes education programs and individualised care plans.
