This opportunity is with our Good Shepherd -Seniors Support Services
Seniors Support Services provides support to older adults (55) in Hamilton who face equity-based barriers such as social isolation poverty and housing precarity that threaten their ability to successfully discharge from hospital to home and/or maintain optimal function and well-being in the community. We have recently expanded this program to include the allied health roles of Physiotherapy Nursing Social Work Physiotherapy Assistant.
The Seniors Support Services is currently seeking a Physiotherapist (PT) to join our interdisciplinary team to support individuals living with chronic diseases and eligible for Assisted Living and home care services. The PT will support a health equity approach to client care in the delivery of evidence-based PT interventions for older adults (55yrs) living in community settings. This will include the assessment of clients physical abilities making physiotherapy diagnosis establishing treatment goals with clients based on their diagnoses developing and implementing physiotherapy treatment plans including therapeutic exercise manipulations massage education electrotherapy hydrotherapy and other modalities as required.
How You Will Make an Impact
The successful candidate will embody the Mission Vision and Values of Good Shepherd by delivering culturally competent respectful and client-centered care within an anti-racism and anti-oppression framework. This role requires providing evidence-based physiotherapy services in accordance with the standards and regulations of the College of Physiotherapists of Ontario (OCPTO) while maintaining strict confidentiality and professionalism.
As part of a multidisciplinary team the Physiotherapist will collaborate closely with healthcare professionals and community partners to ensure comprehensive and effective care planning and delivery. Responsibilities include assessing client needs implementing individualized care plans evaluating outcomes and making necessary adjustments to treatment. The role also involves supervision and mentorship of staff and students as well as maintaining accurate clinical documentation in compliance with privacy legislation and professional standards.
The role also involves prudent use of resources maintaining equipment knowledge coaching and supporting team members and students and participating in staff orientation and training. The candidate will contribute to performance reviews engage in professional development to maintain licensure and competence attend regular meetings and comply with The Good Shepherd Centres policies. Flexibility to work assigned shifts and perform other duties as directed by the Program Manager is essential.
