About the service
Shropshire Community Short Breaks provides Community Short Breaks to families with children and young people aged between 5 and 18 years with disabilities. We currently support up to 200 children and young people.
Our service has different offerings during term time and school holidays.
During term time we run:
After School Clubs offering 3 hours sessions (usually 3-6pm Monday to Thursday) in Shrewsbury.
Saturday Morning Youth Club offering 3 hours sessions (usually 9.30am 12.30pm) in Shrewsbury.
During school holidays we run:
Holiday Club offering all-day sessions (usually 9.30am 3.30pm) 4 days a week in Shrewsbury.
In addition we also offer an Outreach Service all year round covering the whole of Shropshire.
How youll help to create brighter futures
As a Community Short Breaks Practitioner your role is to co-ordinate and direct the support of children young people and families by agreeing interventions which enable positive and agreed outcomes to be achieved. You will help do this by:
