Job Summary: Responsible for providing client care in the field with accurate documentation.
Salary Range: $72000 - $85000 based on experience. $5000 sign-on bonus (with $2500 being paid after 3 months and $2500 paid at 6 months of successful employment).

Essential Functions:
1. Provides comprehensive assessment of patients to determine eligibility and continued eligibility.
2. Reports significant findings to the appropriate physician.
3. Submits documentation in a timely complete and accurate manner.
4. Implements physicians orders in a timely complete and accurate manner.
5. Performs comprehensive coordination of care and case management for each assigned patient.
6. Plans coordinates and implements the patients plan of care in compliance with Agency and governmental regulations.
7. Performs patient care visits in accordance with Agency productivity requirements. Provides patient care delivery in a manner that enhances the overall mission vision philosophy goals and objectives of the Agency. Creates and maintains accurate field charts to facilitate continuity care.
8. Participates in a minimum of 75% of required meetings.
9. Acts as a consultant to other Agency staff.
10. Complies with all local state and federal laws.

Minimum Education: Graduate of an Accredited Nursing Program required.
Minimum Experience:1 year home health care experience preferred and 1-2 years of nursing experience in a health care setting required. Strong communication skills. Strong computer skills.
Licenses/Registrations: Colorado RN license in good standing with the Department of Regulatory Affairs. Up to date car insurance and driver license will be required.

Equipment Used:
Computer based applications such as Microsoft Office Suite electronic health record system accounting system. Office equipment used such as desktop/laptop/tablet computers copiers printers scanners and fax machines.
