Join our team as a Care Coordinator supporting Aged Care services in Rockhampton and surrounding areas. In this role youll manage referrals onboard new clients and adapt care plans to meet changing needs. Youll coordinate home visits and work closely with clients and their families to deliver person-centred support.
You will help to improve day-to-day operations by refining systems and ensuring high-quality service delivery. Your work will ensure clients feel supported and valued while contributing to the ongoing growth of our aged care programs.
This is a 6 Month Maximum Term Full Time opportunity with a flexible location where you can work remotely from home to support our Aged Care Services in Rockhampton. This is a rewarding opportunity for someone with Aged Care experience that is passionate about making a meaningful difference in the lives of older Australians.
