This role combines day-to-day oversight with a strong emphasis on training mentoring and ensuring the consistent delivery of high-quality care communication and documentation across the department.
Responsibilities Include:
Train new and existing Service Coordinators on processes systems and compliance standards.
Provide ongoing coaching and mentoring to improve team performance.
Oversee communication with patients via phone email and required quarterly visits.
Review and ensure accurate and compliant documentation for each case.
Monitor team output and identify areas for improvement or retraining.
Collaborate with leadership and other departments to ensure smooth coordinated care.
Implement best practices and help shape departmental policies and workflows.
Support performance reviews and provide input on team development strategies.
Ideal Qualifications:
Previous experience in a training supervisory or team lead role (preferably in healthcare).
Strong understanding of care coordination or case management processes.
Excellent verbal and written communication skills.
Highly organized detail-oriented and capable of managing multiple priorities.
A collaborative mindset with the ability to motivate and support team members.
Proficiency in basic computer systems and documentation platforms.
