Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted interdisciplinary services in our recuperative care centers emergency shelters housing services and childrens and family programs. IF currently has 13 facilities with 22 micro-communities scattered across Orange County Los Angeles County and the Inland Empire.
Job Description
The Manager of Referral Coordination is responsible for the direct supervision of all Referral Coordinators. This role ensures that all referrals from hospitals and external agencies are reviewed efficiently and meet program admission criteria. The supervisor will coordinate with operations staff to ensure that the volume of accepted recuperative care referrals align with organization goals for facility census. Additionally the Supervisor will develop and maintain relationships with partner hospital referring staff by continuously outreaching to partner hospitals and provide recuperative care referral training opportunities. The supervisor plays a critical role in maintaining operational integrity staff performance and compliance with policies and procedures.
The Pay range for this role is $68640.00 per annum.
Responsibilities
Managerial Responsibilities
Oversee operations and daily workflow of the referral coordination team.
Facilitate daily huddles and monthly department meetings to communicate updates process changes and collect feedback.
Maintain and coordinate staff schedules to ensure continuous operations.
Supervise train and coach referral coordination staff.
Counsel and discipline staff when necessary maintaining professionalism and documentation standards.
Ensure timecards are accurate and schedules are optimized for productivity.
Maintain departmental compliance with contractual and procedural obligations.
Assist with managing referral workflows to ensure timely processing.
Attend cross-departmental meetings and relay important information to the referral team.
This position may require occasional travel to various Illumination Foundation sites meetings or partner locations.
Assist with frontline referral coordination work when staff are out of office and coverage cannot be obtained.
Assist with after hours or overnight calls as required.
Administrative & Operational Duties
Maintain accurate records both electronic and paper based.
Coordinate authorizations and ensure referrals for outside services are scheduled tracked and completed.
Perform statistical tracking of referrals to ensure follow-through and compliance.
Execute administrative projects assigned to meet performance and service standards.
Client & External Communication
Apply customer service principles to interact effectively with patients physicians and office staff.
Develop and maintain professional relationships with referral sources and partner facilities.
Handle communication related to referral completion appointment confirmations and record requests.
Develop and maintain relationships with referring staff from community partners including partner hospitals.
Develop and implement a sales plan marketing recuperative care services to hospital partners.
Continuously outreach to partner hospitals to provide them with information about recuperative care including the referral process.
Directly assist hospital partners with resolving barriers to referrals or threats to the maintenance of a positive relationship when such issues cannot be addressed by program staff.
Technology & EMR Management
Utilize EMR and scheduling software to process and monitor referrals and service orders.
Document appointment statuses using consistent and clear communication.
Scan and upload all related documentation to the electronic medical record system.
Compliance & Organizational Values

Preferred Experience/Minimum Qualifications
High school diploma or equivalent.
Minimum of 2 years of experience in healthcare coordination referrals or a related administrative role.
Previous supervisory or team lead experience.
Proficient in using EMR systems scheduling platforms and Microsoft Office or Google Workspace.
Excellent organizational communication and time management skills.
Demonstrated ability to handle confidential information with discretion.
Valid CA drivers license and eligibility for company vehicle insurance.
Preferred
Associate or bachelors degree in health administration Social Work or related field.
Previous experience in sales marketing or managing call centers.
Bilingual in English and Spanish.
Experience working in nonprofit or community health settings.
Familiarity with HIPAA compliance and referral authorization processes.
Experience with performance tracking or analytics tools.
