As a Social Care Navigator you will collaborate with individuals and families to identify health-related social needs (HRSN) link them to available community resources and address barriers to accessing services fostering stability and empowerment.

Qualifications:
A high school diploma or equivalent along with a minimum of four years of experience in social work case management or care coordination is required. An associates degree in human services social work psychology or a related field is preferred and can substitute for two years of the required experience. Intermediate PC skills including database entry word processing and spreadsheet skills and experience with social services or benefits administration is required. Bi- or Multi-lingual skills are a plus.
