Job Description

Who We Are Looking For:

Are you passionate about social justice and ready to make an impact If so we have an amazing opportunity for you! We are looking for a Housing Access Navigator to join the team at our Assisted Living Program. The Assisted Living Program (ALP) helps people disabled due to HIV/AIDS who are homeless. ALP helps individuals obtain a housing subsidy secure an apartment and then provides ongoing case management to help each individual maintain their health housing and independence.

Schedule: This is a full-time in office position 9:00 AM - 5:00 PM.

Salary: $48000

What Youll Do:

  • Collect required documentation for new clients including HIV status income verification homelessness status insurance coverage and other eligibility documentation.
  • Assess needs of new clients develop Individual Service Plan for each client and assist clients with housing stabilization
  • Enter all client data in appropriate data management systems including eHana and Health Law Institutes ProLaw data collection system in a timely and accurate manner.
  • Assist clients in accessing housing resources including assisting in completing documents for Housing Works database JRI housing waitlists and applying for affordable housing opportunities when available.
  • Participate as a member of JRIs Health Law Institute working closely with attorneys and paralegal. Attend team meetings and collaborate on client issues.
  • Assist attorneys with advocacy and litigation including drafting housing search logs documenting housing conditions issues testifying on behalf of clients when appropriate and other tasks as necessary.
  • Search out available community resources and supports. Advocate and make appropriate referrals for assistance and support. Provide follow-up to individual clients around referrals. Collaborate with other service providers and case managers to ensure continuity of services.
  • Network with landlords community groups advocacy groups and other stakeholders to locate available and affordable housing opportunities in the Boston area.
  • Review and process rental assistance applications for Homelessness Prevention Rent Start-Up Utility Assistance and other potential assistance types. Collect all required eligibility documentation maintain communication with referring providers and collaborate with Housing Access team to provide the appropriate assistance based on assessed client need.
  • Utilize electronic medical record system to keep accurate notes of all interactions with and pertaining to clients and complete required reporting documents. Respect and protect the confidentiality of these and all other records relating to clients.
  • Other duties as assigned by the Program Manager.

Requirements:

  • Must be 21 years or older.
  • Bachelors Degree preferred or equivalent human services experience.
  • Minimum of three years experiencein human services.
  • Experience working with homeless populations preferred.
  • Must demonstrateteamwork and communication skills.
  • Must be able to work independently and manage multiple projects at once.
  • Background and driving record checks will be performed.
