POSITION SUMMARY:
Under general supervision from the program manager the Team Coordinator plans supervises coordinates and directs the daytoday operations of the Public Health funded HIV Case Management team with commitment to providing the highest quality of service to patients. Plans and organizes the work and daily assignments of the support staff coordinates and prioritizes workflow and implements appropriate systems and procedures to maintain service standards.
Promotes continuity of care; resolves operational issues and problems Trains coaches evaluates and disciplines if necessary assigned staff. Ensures appropriate clerical and support staff coverage. Assists with hiring of staff and monitors performance of staff for quality assurance and address training gaps. Conducts all activities within the standards and regulations of the Massachusetts Department of Public Health (MDPH) Center for Disease Control and Prevention (CDC) and any other funders.

ESSENTIAL RESPONSIBILITIES / DUTIES:
Oversee and manage schedule coverage and workflow for medical case management team
Individual supervision to team members to maintain effective patient care based on comprehensive acuity assessments.
Act as liaison between clinical providers and case managers
Responding to the EMR messaging system in a timely manner
Able to think critically and logically when handling patient information particularly around newly diagnosed HIV and all other positive patients
Respond in a timely manner to deadlines and program management
Responsible for following up and tracking any cases until there is a resolution
Tracks and monitors referral from various client referrals sources.
Be able to report basic level data to program management
Attends meetings as requested by the program manager to represent BMC as needed.
Attends staff meetings and communicates progress and problems to Program Manager.
Attends clinical supervision staff support group. Participate in regular public health program implementation team meetings.
Participate in CDC/MDPH/BPHC meetings with other grantees if requested.
Remains knowledgeable of and follows appropriate policies procedures and work standards including but not limited to those regarding hours of work absenteeism and tardiness.
Maintains competency in appropriate areas of responsibility to ensure quality of care.
Follows established hospital infection control and safety procedures
Utilizes hospital values as basis for decision making and to facilitate BMC missions.
Models the ability to set professional boundaries.
Maintains competency in appropriate areas of responsibility to ensure quality of care.
Remains knowledgeable of and follows appropriate policies procedures and work standards including but not limited to those regarding hours of work absenteeism and tardiness.
Performs other duties as assigned or as necessary.

JOB REQUIREMENTS
EDUCATION:
Bachelors degree preferred or High School Diploma and 34 years of experience in the field of Public Health or Case Management or equivalent related experience
However an equivalent combination of education and experience which provides proficiency in the areas of responsibility may be substituted for the education and experience requirement
EXPERIENCE:
Requires 34 years of fulltime experience in public health or working with highly vulnerable populations or equivalent related experience
Supervisory experience in a healthcare or medical office setting is strongly preferred.
KNOWLEDGE SKILLS & ABILITIES (KSA):
Demonstrated technical competency using standard hospital computer systems including Microsoft Office (Word Excel Outlook) web browsers and hospital registration systems (such as IDX SDK Logician etc.).
Excellent English grammatical skills with the ability to communicate effectively both verbally and in writing.
Excellent interpersonal organizational and management skills including the ability to set priorities efficiently delegate and motivate staff to complete assigned work.
Ability to analyze and solve problems in a systematic logical and timely manner.
Must be able to maintain strict protocols of all confidential or sensitive information.
