This position is a member of the Visiting Nurse Associations (VNA) management team and is responsible for oversight of hospice liaisons community liaisons and the agencys intake and registration department. The manager is responsible for assuring that the teams provide service in an effective and efficient manner and that responses to all inquiries for the VNAs services are processed in a timely manner. Manager promotes and maintains positive relationships between referral sources and the VNAs liaisons and intake teams.
The role also oversees the coordination of activities and events that serve to promote the VNAs services throughout community
ESSENTIAL FUNCTIONS:
- Manages the hospice liaisons. Ensures effective and efficient response to all hospice inquiries.
- Manages the agencys community liaisons.
- Oversees daily operations of the Intake and Registration team to assure that referrals are processed in a timely manner.
- Oversee all hiring training and dismissal if appropriate of clinical resource and intake team and all liaisons to meet all the referral needs of the agency.
- Monitors daily weekly and monthly referral metrics that are meaningful and contribute to continuous performance improvement.
- Maintain relationships with referral sources to increase loyalty to the VNA of St. Lukes and its brand identity.
- Responsible to adhere to all federal state and accreditation rules as they relate to referrals for the VNAs programs.
- In cooperation with all VNA departments responsible for service recovery activities for internal and external customers.
- Maintains confidentiality of all materials handled within the Network/Entity as well as the proper release of information.
- Complies with Network and departmental policies regarding issues of employee patient and environmental safety and follows appropriate reporting requirements. Demonstrates/models the Networks Service Excellence Standards of Performance in interactions with all customers (internal and external).
- Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes Customer/Service Improvement Operational System/Process and Safety.
- Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures processes and practices.
- Complies with Network and departmental policies regarding attendance and dress code.
OTHER FUNCTIONS:

- Ability to work as part of the VNA Management Team
- Ability to communicate effectively both orally and in writing to patients families physicians referral sources and related parties.

MOST COMPLEX DUTY: daily management of the liaisons and intake and registration departments to meet the needs of various referral sources. Dealing with service recovery issues.
SUPERVISION (RECEIVED BY AND/OR GIVEN TO): Received from VP Patient Care Services Home Health with a dotted report line to VP Patient Care Services Hospice. Given to Liaisons Intake and Registration staff.
COMMUNICATIONS: Physicians patients families staff and referral sources
QUALIFICATIONS (MINIMUM) EDUCATION: Bachelors Degree from an accredited college or university in Nursing. Current and active Pennsylvania RN license
TRAINING AND EXPERIENCE:
Minimum of 3 years in a registered nurse role. Management experience preferred.
- Work requires professional written and verbal communication and interpersonal skills.
- Ability to exercise tact initiative and good judgement in dealing with all stakeholders.
- Knowledge of HH and Hospice CoP and other regulations are helpful.
WORK SCHEDULE:
At least 40 hours work week with variable hours and days depending on the needs of referral sources patient admissions and agencys requirements including occasional offhour events. Requires a willingness to work a flexible schedule.
