This experience is met by service as a Psychiatric Nursing Assistant 3.
OR
Three years of experience directly working with and providing paraprofessional aide treatment and/or rehabilitation services to patients or clients who have been diagnosed with or experience mental illness behavioral or cognitive disorders and/or addiction as part of assisted living independent living or treatment services home or facility setting.
Substitution:
Postsecondary education in psychology sociology mental health and/or chemical dependency may substitute for up to twelve months of the requisite experience (2.67 semester/4 quarter hours equal one month).
Special Note:
Some positions may require the successful applicant to pass a background investigation including fingerprinting as a prerequisite to employment. These requirements will be indicated at the time of recruitment.
Some positions may require the successful applicant to provide proof they are free from active pulmonary tuberculosis at the time of hire. Employees are required to be tested annually. These requirements will be indicated at the time of recruitment.

List of two (2) professional references including daytime phone numbers.
Copies of all performance evaluations that have been received for the last two (2) years if available.
College transcripts (if using education to meet the minimum qualifications). These can also be attached to your application at the time of submission.

If selected for the position and prior to hire applicant will be asked to:
1. Submit to fingerprinting and background check screening.
2. Provide proof of Tetanus Diphtheria Pertussis (Tdap) Mumps Measles Rubella (MMR) and Varicella Immunizations.
3. Provide proof of Hepatitis B (immunity or declination statement)
4. Provide annual Influenza vaccination or a signed declination statement.
5. Proof of Tuberculosis screening within past 12 months.

The successful candidate must pass a comprehensive background investigation which includes a state and FBI fingerprintbased background check.
