SUMMARY
The Director of Service Coordination supports the CHS mission by managing and directing daily operations of the Service Coordination department and follows the contracts between the Department of Mental Health Division of Developmental Disabilities and Senate Bill 40 Boards.


ESSENTIAL DUTIES AND RESPONSIBILITIES


Affirms the Success of Team Members assigned by recruiting interviewing hiring scheduling and providing necessary training. Planning assigning and directing work by appraising performance by evaluating direct reports for skills directly related to their job descriptions and provides training reinforcement or redirection as necessary. Reward resolves and address complaints or grievances by handling discipline and termination of assigned team members in accordance with company policy by implementing measures focused on motivation of the Service Coordination team to improve performance quality and efficiencies.


Coaches and mentors direct reports on department direction mission and essential job responsibilities by working with SC Supervisors in planning assessing and determining needs for persons served within the assigned geographical areas. Provides guidance and direction as needed to the SC teams by locating residential services forepersons served addressing critical incidents support monitoring issues as well as identifying trends and quality assurance issues with provider agencies as necessary. Manage caseload intakes and transfers within the SC Department for the assigned geographical region.


Creates a Culture of Communication by interacting with staff that promotes the concept of teamwork presents a positive attitude and demeanor maintaining effective communication and professional relationships when working with persons served parents/guardians coworkers administrative staff community agencies and all CHS staff.


Monitors the SC Department budget (for the specific geographical region) by assuring that billing targets are met for the projected budget income expenses are monitored to assure they are staying within the budgeted/allotted amounts. Monitor utilization review data by assuring timelines are being met according to the designated outcomes and troubleshooting/problemsolving any areas of concern.


Delivers Quality Services to Individuals by managing and evaluating the Service Coordination Department (within assigned geographical area) to assure that DMHDDD directives and policies Medicaid Waiver guidelines Individual Support Plan Reviews person centered planning guidelines CHS policies and CARF standards are being met. Maintains a professional working relationship with the DMHDDD Regional Offices by coordinating and communicating provider information to Regional Office Provider Relations and/or Quality Enhancement staff regarding the status of community providers.


Monitors CHS SC dashboard data by identifying trends in performance metrics and quality indicators and addressing these with Supervisory staff as needed.
Responsible for promoting a positive Safety Culture is expected and necessary to make CHS a safe place in which to work. Safe work practices and policies for this position shall be reviewed and followed.


Carries out special assignments or duties when requested by representing the SC Department on various special committees and teams as needed. Attends and participates in staff meetings inservices and staff development training sessions as appropriate.

QUALIFICATIONS

REQUIRED EDUCATION
Bachelors degree (BA or BS) in related field from an accredited university plus seven years of related work experience or combination of education and experience to equal eleven years.


REQUIRED EXPERIENCE
Previous supervisory experience is required; working knowledge of vocational rehabilitation community employment services strong leadership qualities personnel management and exceptional communication interpersonal and organizational skills are preferred.
Excellent Excel Access Word and PowerPoint skills.
Strong communication skills both written and verbal.


REQUIRED MISCELLANEOUS QUALIFICATIONS
Must provide valid drivers license.
Must provide proof of personal valid vehicle insurance.


PREFERRED EXPERIENCE
Experience as a member of a multidisciplinary team in the areas assessing developing or implementing individual personcentered plans especially for persons with intellectual/developmental disabilities.
