Salary: $52500

Catholic Charities of Baltimore Villa Maria Behavioral Health is currently seekingFamily Care Coordinatorwho will work with a child and their family when there is a significant impairment in the childs functioning in one or more of the domains of school family and/or community. The FCC uses a family engagement tool to identify strengths and needs of the family and eventually creates a care plan with goals the family helps to create. The FCC supports the familys pursuit of these goals using their knowledge of the community and support agencies as well as motivational interviewing skills with the goal of improving child and family functioning in the domains identified. The FCC may also provide direct intervention to youth in a group format to practice and support interpersonal skill development. The work schedule will be: MondayFriday 9am5pm 37.5 hours per week(Candidate must have flexibility in scheduling to accommodate after school appointments)

JOB DUTIES & RESPONSIBILITIES:

  • Conducts a family needs assessment and aligns with the family to develop a care plan or an IRP (Individual Rehabilitation Plan).
  • Uses a motivational interviewing approach with the youth and family as they pursue goals.
  • Coordinates meetings and communicates both verbally and in writing with outside providers especially schools.
  • Uses knowledge of local family supports and helps the family to access these supports which can include supports for basic needs medical and transportation.
  • Keeps a written record of work in particular the family assessment and ongoing meetings with family members or providers. This will include documentation in the electronic medical record such as contact notes and treatment plans.
  • Provides updates of family progress as needed including quarterly reporting.
  • Perform other duties as assigned.

EDUCATION & EXPERIENCE REQUIREMENTS:

  • High School/GED and 2 years of work experience with emotionally disturbed children and adolescents and their families.

OR

  • Bachelors degree in social work Psychology or related field and one year of work experience with emotionally disturbed children and adolescents and their families

REQUIRED SKILLS & ABILITIES:

  • Shows reliability by being punctual to meetings with family members and outside agencies.
  • Excellent relational skills with courteous demeanor toward families and providers.
  • Can develop a schedule in collaboration with their supervisor to meet the needs of a caseload of 1215 families.
  • Ability to remain calm in stressful situations.
  • Ability to engage families who are experiencing setbacks and discouragement.
  • Excellent communication skills including active listening.
  • Serviceoriented and able to work independently.
  • Requires the ability to utilize computer systems and software necessary to perform position functions. Basic Windows PC web browsing (i.e. Chrome Internet Explorer etc.) and Microsoft Outlook skills required. Other Microsoft Office application knowledge such as Word Excel PowerPoint Skype and OneDrive desired.

PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:

  • Moving about to accomplish tasks or moving from one work site to another.
  • Communicating with others to exchange information.
  • Hearing and vision abilities within normal range (corrected) to drive safely and to observe and communicate with others.
  • Driving (Agency and/or personal vehicle) is required for this position. Driver must be at least 21 years of age with a minimum of 2 years of driving experience and a valid drivers license issued by the state of residence. There can be no stateissued restrictions on the licenses that would impede a drivers ability to operate the vehicle as required by the Agency and there must not be more than three (3) points on their driving record.
