Job Description

Country Manors Health Care & Rehab Center is looking for a Life Enrichment Associate to join our funloving residentfocused team! In this role youll help plan and lead engaging activities that bring joy purpose and connection to the lives of our residents especially those in Long Term Care.

From crafting to games enjoying visits from Kids Country students storytelling scenic drives and everything in between no two days are ever the same. Youll work closely with our Activities Director and team to create meaningful moments that support the social emotional cognitive and physical wellbeing of each resident.

What youll do as a Life Enrichment Associate:

  • Conduct initial interviews and assessments to get to know residents and understand their recreational interests and needs.
  • Develop personalized Life Enrichment plans that reflect each residents abilities preferences and goals.
  • Document assessments attendance progress notes and care plan updates and communicate important information to the Life Enrichment team and other staff.
  • Monitor and support team members to ensure care plans are followed and activities are meaningful and effective.
  • Assist in designing and coordinating a variety of programs events and monthly activity calendars.
  • Provide individualized visits and oneonone engagement for residents who prefer or require more personal interaction.
  • Help ensure quality standards are met by participating in audits and ongoing program review.
  • Collaborate with the Activities Director and Volunteer Coordinator to plan and carry out special events.
  • Present documentation and insights to the Social Services team in preparation for care conferences.
  • Take on additional responsibilities as needed to support residents and your team.

Qualifications:

  • High school diploma (a degree in Therapeutic Recreation or a related field is a big plus!)
  • Genuine interest in working with older adults and individuals with physical or cognitive challenges.
  • A creative energetic spirit and a knack for coming up with fun meaningful activity ideas.
  • A caring nature and the desire to build strong positive relationships with residents.
  • Strong communication skills both written and verbal and the ability to connect well with residents families visitors and coworkers.
  • A teamplayer mindset and the ability to collaborate across departments.
  • A strong commitment to upholding Country Manors mission values and communityfocused culture.
  • Physical ability to stay on your feet lead activities and assist residents as needed.
  • Strong time management and organizational skills.
  • Basic computer skills (or a willingness to learn) for documentation and planning.

This is a fulltime position Monday through Friday from 9:00 AM to 5:30 PM no weekends!

We do ask for availability on two holidays per year to help us celebrate with our residents!
