There is a market supplement of 10% to the base salary to all staff who are employed in night roles. This will be paid three times a year.

We have a new opportunity to join our team to:
Take referrals into our service by phone and Liquid Logic (our social care system)
Deploy support workers from the Swifts and Nightowl services to unplanned events such as falls or welfare checks
The role may also include setting up visits to service users for support workers using an electronic scheduling software (full training will be given)
Take queries from the support workers while they are working in the community
As a Reablement Liaison Officer you will be the first point of contact and an information service for GPs community health services social services service users and their families. You will also take referrals for Norfolk First Support.
Norfolk Swift Response service provides support for unplanned care needs such as falls and is a 24hour service. It is part of Norfolk First Response which delivers services to adults across Norfolk in their own homes for up to 6 weeks assisting them to regain skills they may have lost following an illness or hospital admission enabling them to regain their independence as much as possible while also reviewing any ongoing support needed.
You need to possess excellent ICT skills and be able to work to tight deadlines often making decisions independently whilst delivering excellent customer service. You will also have the ability to assess risk and take appropriate action working with our multidisciplinary team to support people to return to their homes.
The role is hybrid working both from home and a locality rota is on a twoweek rota basis. Shifts are 9:45pm to 6:15am and include bank holidays on a rota basis. There may be the requirement for weekend working on an ad hoc basis on order to support the service.
