Developmental Pathways (DP) has an opportunity for a Senior Case Manager (SCM) Active in our Case Management Department.The Active SCM will be responsible for verifying eligibility and developing monitoring and assuring implementation of the Service Plan (SP). The Active SCM will manage a caseload of individuals receiving waiver services and maintain documentation needed quarterly and annually. The Active SCM will facilitate interdisciplinary teams (IDT) meetings to coordinate services to meet individual needs; ensure appropriate interaction and communication; and implement IDT meetings as appropriate to modify services. Lastly will provide case management services and support to temporarily assigned individuals; support with department and team coverage as needed and provide training and mentorship for Active Case Managers.
This is a 40hour per week hybrid position with the benefit of working remotely and inperson coming into our headquarters office in Aurora CO and/or offsite locations.

- OR Five (5) years of experience in the field of LongTerm Services and Supports (LTSS) which includes Developmental Disabilities;
- Or some combination of education and relevant experience appropriate to the requirements of the position
Must have demonstrated exemplary performance while working within service coordination or case management positions including the following areas of performance: demonstrated initiative within the department or agency on projects demonstrated leadership skills adaptation to change and problemsolving skills.
Valid Colorado motor vehicle license & proof of motor vehicle insurance
Ability to meet and maintain agency driving requirements
Upon request qualifying need and approval driving requirements may be waived in lieu of reliable transportation.
Preferred
- Knowledge of rules and regulations for Health Care Policy and Financing and the Colorado Department of Public Health and Environmen
- Two (2) years of experience in service coordination/case management at Developmental Pathways or similar agency
- Familiar with Microsoft Office (Excel Word Outlook etc.) and be able to use these and other required programs at an intermediate level
- Able to speak write and interpret fluently in English and another language based on the business needs (you may identify if its Spanish specifically or what the need is here)
- Ability to complete all typical service coordinator responsibilities in fluent language/s
- Support with written translation of internal documents for use by stakeholders as appropriate
Additional Responsibilities
- Assure services and supports identified in the annual service plan are coordinated among various providers for continuity of services and funding for individuals served. Facilitates and coordinates Interdisciplinary Teams (IDTs)
- Provide leadership and mentoring to Case Manager positions and problemsolve complex situations with the management team families and providers
- Communicate in an appropriate effective and respectful manner with individuals served supervisors other staff agencies and organizations; maintain individuals served confidentiality
- Maintain a professional and ethical manner in all interactions and meet performance quality customer service and coordination standards as assigned by department leadership.
- Serve as subject matter expert for applicable programs and services; serves as resources/point of contact for complex cases
- Complete various assessments to determine eligibility for services/supports and identify individual strengths needs concerns and preferences through interviewing observing and utilization of standardized tools
- Provide care coordination services and interventions by referring educating collaborating and mediating with individual and external providers/agencies
- Complete accurate case documentation in appropriate tracking and databases; prioritize workload and meet all required timelines
- Complete required documentation related to monitoring activities and other targeted case management activities as PMPM; evaluate need and effectiveness of services requested and delivered; advocate for individual service needs
- Schedule facilitate document and complete followup for all required meetings; disseminate updates and obstacles to the necessary parties
- Ensure the accuracy and timely entry and distribution of required documentation to all necessary parties including but not limited to providers individuals and family member
- Maintain knowledge of state/federal regulations policies and procedures regarding longterm services and supports Medicaid waiver and statefunded programs and other community resources
- Provide oversight and coaching to case management level staff regarding support for people in services conflict resolution and negotiating positive outcomes for the Department and people in services
