Job Description

This role is vital in creating a supportive rehabilitative environment for individuals recovering from illness injury or surgery.
Key Responsibilities:
- Conduct thorough physical assessments and review medical histories to evaluate patients conditions.
- Develop and customize treatment plans tailored to each patients specific needs and objectives.
- Implement a range of therapeutic exercises and training programs to enhance patient recovery.
- Provide education to patients and their families on injury prevention and wellness strategies.
- Monitor and document patients progress towards achieving their rehabilitation goals.
- Collaborate with other healthcare providers to ensure comprehensive and holistic patient care.
- Maintain accurate and uptodate patient records in compliance with regulatory standards.
Qualifications:
- Masters degree in Physical Therapy.
- Minimum of 1 year of experience in home health as a physical therapist.
- Current licensure to practice as a physical therapist in the state of operation adhering to professional standards.
- Valid CPR certification required throughout employment in a clinical role.
- Must possess a valid drivers license and the ability to travel to various business locations.
