About Strive

Founded in 1996 as Landsdale Family Support Association Inc. our original purpose was to provide respite camps for children with disabilities at Landsdale Farm School. In 2009 we changed our name to Family Support WA Inc as we expanded our services to include young adults. Over the last 29 years we have grown to provide several services to people of all ages with disabilities as well as now operating the Landsdale Farm located in the northern metropolitan suburb of Darch. We are now called Strive Together WA Inc. which reflects the wide range of services we provide in the community.

About the opportunity

Are you passionate about making a meaningful difference in peoples lives We are looking for passionate support workers who bring empathy enthusiasm and a strong commitment to personcentred care to work in our supported independent living (SIL) homes in Landsdale. We believe the ideal candidate brings a combination of proven experience with challenging behaviours a relevant disability sector qualification and a deep connection to our values vision and purpose.

If youre passionate qualified and purpose driven we want to hear from you. We offer thorough training and ongoing support and were especially interested in candidates with experience in with nonverbal clients high behavioural needs personal care and mealtime management.

This is a permanent parttime position with opportunities for career development.

Key Responsibilities:

  • Implement personcentred support plans prioritising client safety and wellbeing.
  • Coordinate appointments and manage clients admin financial and health needs in line with organisational procedures.
  • Provide assistance with daily living tasks including medication administration domestic duties and personal care needs.
  • Promote collaboration and continuity of care through clear timely communication and active participation in forums and decisionmaking.
  • Support client wellbeing through collaborative meal planning and nutrition education tailored to individual needs preferences and budgets.
  • Maintain meticulous records documentation and case notes to ensure progress tracking and effective communication.
  • Uphold the highest standards of integrity confidentiality and dignity in all interactions.
  • Cultivate positive relationships with clients and their support networks.

Selection Criteria:

  • NDIS Check Working with Children Check and current first aid certificate (or willingness to obtain all three before commencement).
  • Certificate III in Individual Support or Certificate IV in Disability.
  • Prior experience in supporting NDIS participants in community access or SIL accommodation.
  • Valid WA Drivers License and a roadworthy vehicle.
  • Alignment with Strives organisational values vision and purpose.
  • Excellent communication skills both written and verbal.
  • Previous experience with writing detailed case notes for clients.
  • Current First Aid Certificate (including CPR) or willingness to obtain one before employment commencement.
