We are currently recruitment for an experienced Social Worker to join out community palliative care team based at St Vincents Private Hospital Brisbane. This newly created position is a fixedterm 12month opportunity working 2 days per week.

Things that work for us:
Ability to work on fixed term 12month basis working 2 days per week
Relevant Bachelor degree
Australian Association of Social Workers (AASW) Membership
Demonstrated experience in Social Work preferably within Palliative Care
Demonstrated experience in providing a service which enables patients and their families to deal with the impact of changes in health on individual and family functioning
Ability to ensure prompt assessment of newly referred patients
Ability to work closely with the multidisciplinary team involved in the care of patients and their families
Ability to participate in and support education activities and to contribute to the development of self and other team members
Have a working knowledge and understanding of relevant regulations and legislation
