Program Description The Shortterm Adjunctive Youth and Family Engagement (SAYFE) program is a comprehensive full service partnership (FSP) for 40 of the highest risk children/youth living in San Mateo County. SAYFE is designed to help children and youth achieve independence stability and wellness within the context of their culture community and family/caregiver. SAYFE seeks to stabilize youth in the community using natural support structures and through working in collaboration with San Mateo County. Position Description The Case Manager is member of our multidisciplinary team whose role is to serve parents caregivers and nonprofessional supportive adults of our youth (ages 618). This individual helps coordinate the treatment of youth and families among internal and external (which may include: schools mental health physical health/medical juvenile justice and/or legal child welfare systems) providers.

Essential Functions - Provide counseling and behavioral interventions guidance skillsbuilding and support in accordance with each clients individualized treatment plan and the programs treatment philosophy and model.
- Provide services to 618year olds and their families at Edgewood locations in the home at school and/or in community settings as required by the treatment plan.
- Engage family members and caregivers as full partners in service planning delivery and evaluation.
- While under the Case Managers care maintain supervision of clients at all times to ensure their safety and wellbeing.
- Ensure facilities living quarters office spaces equipment and furniture to be used in providing services are in good working order and professionally maintained at all times.
- Engage in crisis interventions and deescalation including the application of conflict resolution and collaborative problemsolving techniques.
- Implement clinical policies and procedures and actively enhance communication coordination and integration of care with other Edgewood service areas and with external providers.
- Contribute to organizationwide quality management and improvement efforts by consistently providing the highest quality professional interventions fulfilling all documentation requirements and proactively seeking ways to enhance the quality of services and surpass minimum required standards.
Minimum Qualifications - High School diploma or equivalent required; Bachelors degree preferred.
- Equivalent education and relevant experience may be considered in lieu of a bachelors degree.
- Bilingual SpanishEnglish required.
- Demonstrated ability to work with families in a wide variety of cultural contexts and community settings.
- Must have a valid California drivers license and proof of insurance.
Salary Information This position offers an hourly rate of pay of $31.90 ( per hour withbilingual language skills). *Please note the language differential is contingent on passing a language proficiency test.
