ProMedica Toledo Hospital is one of the largest acutecare facilities in the region. REPORTING RELATIONSHIPS/SUPERVISORY RESPONSIBILITIES

This position reports to Director ProMedica Pharmacy Medication Management.

POSITION SUMMARY

Under the direction of the Director of ProMedica Pharmacy Medication Management performs ambulatory care clinical pharmacist functions necessary for the daily operations. The clinical pharmacist must be willing to cover at any ProMedica Pharmacy Medication Management site at any time as deemed appropriate.

ACCOUNTABILITIES

  1. In collaboration with providers manages referred patients medication therapy as outlined in the Collaborative Practice Consult Agreement.
    1. Communicates politely effectively and professionally with patients other pharmacists and all other healthcare providers verbally inperson and via phone as well as written through mail and electronic medical record (EMR).
      1. Clarifies any medication questions with any of the patients providers/specialists when needed.
      2. Cooperates with management and peers to promote an environment of teamwork and collaboration.
    2. Performs pointofcare tests and/or orders disease related tests based on referred patients medication therapy; documents results and communicates results following policies and standards of ProMedica clinical laboratory.
    3. Performs outpatient chart review for appropriate medication usage and provides patient counseling about related drug therapies.
      1. Monitors drug therapy regimens for contraindications drugdrug interactions drugdisease interactions drugfood interactions druglaboratory test interactions allergies appropriateness of therapy and therapeutic duplications.
    4. Utilizes standard work as well as disease state and medication assessment per evidence based care plans protocols and clinical judgment.
    5. Using EMR documents all communications/activities/interventions accurately and thoroughly and directs any messages as needed to the appropriate person.
    6. Using EMR sends Standing Order/Confirmation of Verbal Order/specific request to a lab home health agency extendedcarefacility physician etc. as needed.
    7. Using EMR schedules/reschedules patient appointments for the pharmacist visit and submits accurate charges for patient encounters.
  2. Contributes to education of pharmacy residents pharmacy students and medical students as assigned.
    1. Educates staff prepares patient educational handouts and reviews current materials and drug information.
    2. Participates in medication related research and projects.
  3. Participates in quality improvement activities required by the Joint Commission ProMedica hospitals ProMedica Pharmacy Medication Management Quality Review Teams or others as needed. Demonstrates compliance with licensing regulatory and accrediting agency provisions.
  4. Complies with established departmental policies and procedures objectives quality assurance programs and safety standards. Evaluates current policies for review and revision as necessary.
  5. Attends orientation education and training programs. Reviews literature and other materials as appropriate for the position. Attends mandatory staff meetings and performs other related duties as assigned or requested.


Requirements

REQUIRED QUALIFICATIONS

Education: Must be a graduate of an Accredited School of Pharmacy (ACPE) and have a Doctorate of Pharmacy (PharmD).

Skills: Must be able to understand directions communicate and respond to inquiries effectively and timely requires effective interpersonal skills. Must function as an integral member of the team. Must be able to input and retrieve information from the computer.

Years of Experience: Must have completed a PGY1 (Post Graduate Year 1) pharmacy residency (for all new hires) or at least two to three years of hospital pharmacy experience (for any previous hires).

License: Must be an actively registered pharmacist (RPh) licensed in state of practice primary source online verification required directly from State of Ohio Licensing or State of Michigan Licensing database. Must obtain other state license within 1 year of hire.

Certification: Must have a certificate of registration and a current identification card in state of practice. Must maintain Basic Life Support certification.

WORKING CONDITIONS

Personal Protective Equipment:Must wear appropriate PPE. Must wear gloves and suggested to use protective eye wear when performing pointofcare testing.

Physical Demands:Must be able to lift light materials. Nonmaterial handling of continuous sitting and reaching; occasional standing squatting kneeling walking and stairs. Must be able to move about and between workstations. Must be able to visually tolerate exposure to blood.
