Overview
Target Housing is looking for a Case Worker within South Yorkshire to work within our Sheffield complex needs project. The purpose of this role is to provide high quality holistic support to our clients. As a Case Worker you will lead on support for your allocated caseload of clients coordinating with others internally and externally as required. You will work with your clients to enable them to develop their knowledge skills and confidence so that they can increasingly make their own decisions and take more responsibility for their own lives and communities.
Job role
You will provide high quality housing management and support services to clients in order to assist them in sustaining their tenancies achieving their potential and working towards independent living. You will be working with clients on a 1:1 basis you will transport them to appointments will oversee and coordinate support for allocated clients including completing risk assessments support plans and safety plans. You will also monitor clients compliance with their occupancy agreements and act to resolve noncompliance issues.
