Job Description

Are you looking for an opportunity where you can make a difference in peoples lives Community Connections Coordinators at TNC Community get to engage in fun community activities while working oneonone with individuals with intellectual and developmental disabilities to help them become more independent.

CCCs help individuals with intellectual and developmental disabilities plan and engage in activities that get them involved in their community encourage them to try new things and ensure they live fulfilling lives.

Responsibilities:

Identify and plan activities that align with supported individuals interests and goals

Encourage and ensure accessible participation for individuals with varying levels of need

Transport individuals to activities and remain actively engaged throughout activities

Document community integration activities for state reporting and marketing

Protect the rights and safety of supported individuals

Schedule: Parttime is late afternoon and evening; weekends

Qualifications:

  • High School or equivalent (required)
  • Associates or higher or equivalent experience (preferred)
  • Drivers license (required)
  • Experience working with IDD individuals (preferred)
  • Experience planning/facilitating community activities (preferred)
