Job Description

PACE Home Care Coordinator HHA

Chula Vista (San Diego CA)

FullTime. Direct Hire.


The PACE Home Care Coordinator HHA is an essential member of the interdisciplinary team (IDT) working under the supervision of the PACE Home Care Coordination RN Manager. This role supports geriatric participants by coordinating homebased care services ensuring their healthcare needs and preferences are met with compassionate personcentered care.


Key Responsibilities:

Conduct home safety evaluations (preenrollment annual semiannual or as needed).

Coordinate IDTapproved services including meal delivery caregiver services and durable medical equipment (DME).

Document and track service delivery in compliance with clinical DHCS and CMS regulations.

Advocate for participants liaise with families and healthcare providers and collaborate with the IDT for seamless care.



Requirements


Qualifications:

Home Health Aide (HHA) and CNA certificates required.

Minimum one year of experience with frail or elderly populations.

Strong communication and teamwork skills; basic proficiency in Microsoft Outlook and Excel.

Valid California Driver s License.


Additional Requirements:

Education: CNA and California Department of Public Health HHA certificates; CPR and First Aid certification (American Heart Association).

Adhere to health safety and organizational policies; participate in meetings.


This role is critical to the Program of AllInclusive Care for the Elderly (PACE) ensuring participants receive highquality coordinated care in their homes.
