Job Description

The inpatient social worker collaborates with the interdisciplinary team to coordinate the care of the patients focusing on what the patient will need post hospitalization. They will assess evaluate and coordinate a safe discharge plan with the patients/families goals of care as their guide. The goal of the Inpatient Social Worker is to transition the patients to the next level of care in a timely and safe way with the wishes of the patients/families at the forefront of those transitions. This professional is responsible for maintaining very effective interprofessional collaboration across the clinical teams involved in patient care as central to the success of the work.

Education

  • Bachelors degree in Social Work psychology or human services field is required
  • Masters degree in Social Work psychology or human services field is preferred

Licensure/Certification

  • LCSW preferred

Experience

  • Minimum of 2 years social work experience preferably in an acute care hospital setting required
  • Training or experience in chemical dependency or mental health environment desirable preferred
